Wandering residents and sleepless nights

I was sleeping soundly Monday night when Maria (pseudonym) pushed her wheelchair through my door. She is mentally challenged and when she cannot sleep, Maria roams the halls. It was 12:38 a.m., so I put my call light on and an aide soon took Maria out.

Alzheimer’s may spread by ‘jumping’ through brain regions, study finds

One of the biggest mysteries involving Alzheimer’s disease may soon be uncovered.

The question has been whether Alzheimer’s starts at independent regions of the brain at different times, or if it spread to neuroanatomically connected areas.

A new study by Columbia University Medical Center (CUMC) researchers strongly supports the latter, demonstrating that abnormal tau protein, a key feature seen in the brains of those with Alzheimer's, propagates along linked brain circuits, "jumping" from neuron to neuron.

The bill that the CLASS repeal was a part of now moves on to the Senate, where Democrats aren’t expected to bring it up.

The percent increase was 2.05 percent in facilities reporting data for both the 2010 and 2011 reports. The report is published in cooperation with LeadingAge and supported by the National Center for Assisted Living.

CBO: 10-year repeal of sustainable growth rate would cost $316 billion

Permanent repeal of the sustainable growth rate, which controls the Medicare fees physicians receive for their services, would cost $316 billion by 2022, according to a new Congressional Budget Office report.

The CBO, which released its Budget and Economic Outlook report Tuesday, said outlays for Medicare would be $9 billion higher in 2012 alone if Congress overrides a scheduled 27 percent reduction under the SGR, which would take effect March 1.

CMS rule would increase Medicaid drug pricing transparency, save $17.7 billion

A new proposed rule on medication pricing transparency would save an estimated $17.7 billion over five years on prescription drugs bought through Medicaid, according to estimates issued last Friday by the Centers for Medicare & Medicaid Services (CMS).

The CMS rule, which implements prescription drug provisions of the Affordable Care Act, would increase transparency in drug pricing to ensure states are not overpaying for prescription drugs.

When residents start food fights

A few weeks ago, a 20-something severely disabled resident, who I will call John, returned to this nursing home. When he lived here before, it was common for him to become agitated, particularly at meal times.

On his first day back, he seemed alert and fed himself. Perhaps he had changed.

Unfortunately, after several days his demeanor turned. He was more sedated, it was harder for him to feed himself and the aides had to assist him.
